Addendum: Advocate Confidential – March 11, 2002 Associate Access to Patient Information Under the federal patient information privacy rules, health-care providers are expected to take “reasonable safeguards” to prevent inappropriate uses and disclosures of patient information. In addition, the privacy rules require that only the “minimum necessary” patient information that is needed to care for of the patient or to perform other work is accessed by staff or associates. This rule applies to all associates and volunteers who work with patients or with patient information and requires that the patient information each associates needs to access be documented. This is done best by including a provision in the associate’s job description that identifies the level of access to patient information the associate requires. All associate job descriptions must be revised to include this provision by March 1, 2003. Managers should work with Human Resources to update all job descriptions and should review their level of access to patient information with all associates, and review the access level with associates annually. Associates need to be aware of the types of patient information, behavioral health information and IS systems they have access to, and report inappropriate system access to their manager. The following template, which will be an attachment to each job description, has been developed to make the required job description changes. Check the appropriate level of access in each of the four parts of form. The level of access refers to access to the paper patient record, as well as any computerized patient information. All four parts of the template must be completed: Part I This positions requires: 1. no access to patient information 2. access to the fact that the patient is present at the facility 3. access to patient demographic information (name, address, phone number, insurance information, etc. ) 4. access to patient demographic and insurance information • And this position also requires access to the following limited clinical information (list specific portions of the record the associate needs to have access to) 5. access to patient demographic and insurance information except ……………… • And this position also requires access to the entire patient clinical record excluding (list specific portions of the record the associate does not have access to) 6. access to the patient demographic and insurance information • And this position also requires access to the entire patient clinical record Part II Behavioral health access, this position: 1. does not require access to patient behavioral health/psychiatric clinical information. 2. does require access to patient behavioral health/psychiatric clinical information Part III This position requires: 1. access to only current information about patients 2. access to only historical information about patients 3. access to both current and historical information about patients Part IV This positions requires: 1. access to patient information located in the following information systems: list all required (Allegra, IDX, CareNet, RadNet, PharmNet, Midas, TSI, Eclipsys Decision Support or Patient Care System, all others Advocate Health Care Position Analysis Worksheet Privacy Addendum to Job Description Job Title: Ultrasound Technologist II Job/Position #: 3AE46 Assessment Date: 5/02 Site: High Tech Medical Park Please designate the appropriate level of access for the job/position description.
